My Lumia 521 volume goes out of whack, any fix?

What happens is whenever I'm listening to music on my headphones, the phones volume goes crazy, going up down and all around. O have the Amazer case for it, the big bulky one, so I thought it was the case messing with the button. I take it off, and try the volume again, and it immediately dipped to 0, back to 30.
Its not like its a new phone, I've had it over a year.

I've never seen this issue before... Does the volume jump around when using the phones speakers and playing music? If it only happens with headphones, I'd be inclined to check of maybe they are the culprit? Do the headphones have a volume adjuster and maybe it isn't compatible or the buttons are stuck or have gotten something in it that is sorting the button s?

The headphones don't have adjusters, and I haven't checked it with speakers yet, I'll wait till I get home to do that.
