You need Lumia Denim. Simply drag one app on your start screen on top of another app on your start screen to create a folder.

If you have Denim or Cyan + D4P just pick up a tile on your start screen and drop it on another tile.
If you are on Cyan down load the app " App Folder" and start it up and follow up the instructions
