Why is there no sound in head phone jack?

I have no soft ware contact to the head phone jack. I have done everythin: changed sim, phones, reset, reboot, different sound files. If I shut down the phone and restart with the phones connected I have excellent sound ... til next time it doesnt work.
I have also lost the vibration.
