Anyone know the APN settings for a Nokia 920 on Ultra Mobile?

I just got a Nokia Lumia 920 activated on Ultra Mobile. The nokia 920 is a windows phone and I tried to get customer service to help me, but they seem clueless. I'm trying to get the apn settings for internet and mms. Thanks!

On the phone go to: Settings > cellular + SIM > SIM settings - THEN - edit internet APN - & - edit MMS APN

Match the values from the link I posted in the fields... If a field is not listed on what they show, leave it blank. If they show a value for a field not displayed on the phone, pass on by.
