Why is my Lumia 925 frozen on lock screen?

I can't get it to turn back on by pressing buttons. The clock is wrong with an old time. It says 7:38 and its now 9:30. This has happened before. I had another 925 and I brought it to the warranty center and got it replaced. Then with my new phone it happened a week and half ago. I thought it was the charger but it can't be. i didn't have my phone plugged in this time when this happened. Las time it wouldn't turn on for 1 and half days. I didn't start having problems till last month.

Have you tried a soft reset? (Hold power button + volume down until phone vibrates)
Also try it with the charger plugged in.
