How much does a dev license cost for Windows/WindowsPhone?

- Is the cost monthly, yearly, or a one time cost?

As of now, it is about $20 USD for the first year... After that, as long as you stay active as a developer (releasing apps and/or app updates) the yearly renewal is free.

Granted, this is for apps that are what used to be referred to as "metro", to be released in the Windows/ Windows Phone stores.
