I have a Lumia 640 on 8.1, why haven't I been offered the upgrade to W10 yet?

I have a Lumia 640 on WP 8.1 update 2, build 8.10.15148.160. I have been waiting for some time since the official launch of Windows 10 for mobile and I haven't been offered the upgrade yet. Despite seemingly many other Lumia 640 owners having had the upgrade. I downloaded the upgrade assistant from the store but this says it is not yet available for my device. Any ideas? Is this a widespread issue?

If you have the AT&T version, you have to wait for them to approve the release first for your phone. If you want it now, sign up for the Windows Insider program and select production ring and you'll have access to the most recent Windows 10 mobile build.

Steven, more details will help us understand better... Is it carrier branded? What branding? If a Country Variant, what country?

Could you look in Settings > Extras & Info, please?

Tell us what it says for Manufacturer Name & also for Mobile Operator.
