Randomly restarting and freezing

Symptoms:-

1) Phone randomly restarts. Sometimes, it keeps vibrating and restarting.
2) Screen randomly freezes. Volume, power and the camera button works. But since it cannot be 'slid down', it cannot be restarted. I have to use the volume down and power button to do so.
3) A very few times, the phone shows wrong battery status-
(i) When connected to the charger, it shows 'Extremely low battery' icon.
(ii) For the different number of times it reboots, it shows a different battery status.
4) The battery does not heat up. There is a lot of memory space available on the phone (3GB). Not a single third-party app.

Things I've tried:-
1) Hard Reset
2) Cleaning up space

As you have tried the obvious already, have you tried the Windows Device Recovery Tool? Also, have you attempted to run any insider preview OS's?

With WDRT you will have a fresh install and then can troubleshoot from there.
