Why can Cortana suddenly stop working?

I have Cortana on my Lumia 635 and it has been working fine, until this week when it suddenly stopped acknowledging me and only "speaks" when directly accessed - "Hey Cortana" for example, but thereafter it does not have a voice response. My settings, who I am by name etc. have all gone and I do not seem to be able to reset the App.

My phone is running Windows 10 build 10.0.10586.107

you can update your phone so that it runs a newer version (10586.318)

Go to settings > update and check for updates and download

It should help in resolving the issue
