Why my battrery drains so fast?

first of all my battery is not charging, the phone shows that it is being charged but it doesn't even add 1% tho the battery and after i remove the charger the phone dies and says battery empty.
I have even changed its usb jack and its software but the problem still persists. is their a problem with my battery. plz help me.........

It sounds like you do have an issue with your battery. It might be worn out.
