Some apps just lost the icons

Since last OS update .1004, I noticed some of my apps just lost the icon in both app list and start screen.

In the app list you will notice some of them don't just have icons.

The start screen has some of them pinned, and you will see there is no icon as well.

More funky things are:

• I have HERE Drive+ pinned on the start screen, but I cannot find it from my app list if I swipe from right.
• My start screen setting is to use my own picture + style = "Tile picture". I have some small areas without any app pinned, they are just pure black. Shouldn't empty areas be displayed as transparent?

All these happened since .1004 was installed.

I've found that this sometimes happens when apps are moved between the SD card and the phone's internal storage. Usually a soft reboot fixes it, but if not then you can try removing and reinstalling the apps to get the icons back.
