Modern app icon bug!

I have a problem with modern app icon, it shows up with a white background, makin it impossible to identifie apps in the taskbar(while in the start menu it look just fine)
also if I do a search in cortana, modern apps icon show up as a generic file icon (white papel icon)

any help? how can I fix this? I'm already downloading the anniversary Update .iso to see if that help.

What is the name of the app? Just simply "Modern" is not an app I can find.

Are you using anything special in settings > personalization > themes?

I mean...all modern app (metro app whatever you want to call it), no special setting, it all happened after I entered the insider program, my account setting or something like that go corrupted, i logged in on a "temporally user" then i had no metro apps, when I installed the anniverary update, the apps were back, but the icons are like that, every single app instea of a transparent background, has a bizarre white square...

You had some icons that look normal next to your example so I had to ask...

Looks like a white "font" or symbol on a white background. Nothing in settings > personalization > themes changes this?
