Yes, that would be excelent for people that doesn't have a credit card, or (like my case. so anytime i wan't to buy an app or game i have to use my mom's) it's credict card only works in their country(in my case i have to pay extra fee to use outside), and think of this: there's a lot people that don't buy apps or games for that simple fact (the lack of credict or debit card) and thus they pirate them (not the case of wp 8, yet), so yeah, i would love if any of my carriers in my home country would implement this feature.

Then blame the carriers not Microsoft as they are the ones that have to implement carrier billing ;-)

That depends on the carrier... I have it, and use it in M?xico (Telcel)
