You need a Lumia 930, 1520 or ICON to record 4K video, and the phone must be updated to the latest Lumia Denim firmware.

Only the snapdragon 800 series supports recording in 4K resolution.

That is not possible.
You need Denim and a 930/icon or a 1520 and use Lumia Camera 5.
And there is a setting in the app for 4K video.

and if you have one of those devices, you go to Lumia Camera (5), hit the settings/gear button, swipe to video, and set the resolution to 2160p and your framerate of choice.
