For starters, what phone brand and model do you have? (e.g. Lumia 640, Blu Win HD, etc.)

Which version of the Windows Phone OS is it running? (e.g. Windows Phone 8, Windows Phone 8.1, Windows Phone 8.1 Update 2, Windows 10 Mobile)

What services are you starting when this occurs? Does anything else cause the phone to shut off? Does it ever shut off while doing nothing or while the phone is being jostled?
