Not that this is an answer to your question, but, I've been running with Bluetooth headphones for the past couple of years and recommend them to anyone who is active with their phones. No wires to get in the way and no concern about poorly made cases.

Those ones appear to have a plastic covering over the screen. Does anyone know of any that keep the screen exposed?

I bought this one:

It doesn't expose the screen, but for what I do (run, walk the dog), its a non issue. I also like that it doesn't require any velcro to hold in place. Its spandex so it stretches around your arm. Just make sure you get one size under what your arm is. I ordered a L with my biceps at 8in. It would constantly slip down my arm.

I bought one for my 4S and it was too small for my arms. I've thought about putting it on my forearms instead... not sure yet.

Not sure, I've only tried the one, but I noticed at the gym... those with big arms don't use them.
