I seem to have lost Cortana, how can I get it back?

I seem to have lost Cortana

I have noticed that the microphone is no longer there, and if I ask a question Cortana doesn't say anything anymore

Jokes aside.. which build are you currently on? (settings>about)

Did you download the speech pack? settings>language

Are you set to the US? as region
