Same error code during an update to Bing Finance, now MSN Money, a few weeks back. I've kept trying, soft reset, blah blah. Finally chatted with someone from Microsoft and the only help was telling me to hard reset.

The way I accomplished it was find the file in Music somewhere, send in an email as a text (ie. xxxxxxxxxx@vzwtext.com or something similar) so it shows up as a text. Then saved it as a ringtone. Disclaimer - this was from my 928 to a simple flip phone.

First picture looks normal and will happen with any camera. Second pictures looks like the lens has an oil smear from your finger on it.

I have never had Cortana pinned to start, have done the Live Lockscreen Beta fix, and even hard reset. I still see resuming or loading plenty of times. It started after the upgrade from 8.0 to 8.1.
