solved, this seems to be NFC problem :) since i keep my cards with my phone, they interract. Disable NFC or remove other cards near phone.

Is there any solution for this problem? I have this thing on for a month or so. Just factory reseted my phone, its still there. It is making the usage of my phone really really hard, everytime its there when i open my lockscreen. Need help on this...!!

Yes, I have same problem. Just factory reseted my Lumia 950 and the problem still occurs. It is really annoiyng, every time i open my lock sreen, its there, sometimes multiple times. Push Ignore, it goes away til i make a call or open my phone again. Push Open app, nothing, just goes away...
