Did this happen after a recent update? Like daimv stated, sometimes it can take a moment before it's recognized? (lots of files for the USB to page through = leave it plugged in and have longer patience before expecting it to show up as navigable media)

It is, but in my experience it isn't something that happens "on it's own," it's more of an operator error. Unless of course, you weren't trying to root it. Then, this behavior defies explanation.
