It's sold on Amazon and Ebay, I got mine on a trip to India. So far I love it, I like to keep my things personal and helps a lot in the nights or the cinema, nobody has noticed I'm using a phone (blocks/reduces brightness from every angle except yours)! Lowers the brightness and the quality a little bit, touch responsiveness is great and aesthetically it looks good with my black lumia and a black case, somehow it hides the nokia logo... It's thick and the screen can only be seen by the person who is holding the phone... if you like to share a lot what you are viewing on your phone, this is not for you! The angle of view is little!
Now, going back to the topic of this thread.
For the titles with icons I use an app called Group Tiles from the Window Store. They are just tiles!
You can set colors of anything to foreground (theme accent), background (tiles background image) and obviously any color you can think of. It has some pretty cool icons, styles, sizes... very personalizable, just play with it!

For the background color on tiles, just use any image with the color you like...
For transparent tiles, use this app Transparency Tiles. It's free! It has most used app tiles, there's also a paid version which I guess has a lot more of app tiles. I have never used it.

For weather I use Blue Skies, I like the live tiles! Useful, simple, accurate! Doesn't drain battery.

For the clock I use TimeMe Tile, you can personalize it with events from calendar, alarms, weather, etc... pretty much everything!
