SD error

SD card was working fine until I rebooted the phone and got message that SD card might have issues so I should scan it.
After scan finishes phone doesn't recognize it anymore . Is it SD card or phone as tested the card in another phone and it is fine .

Can you read the SD card on your computer? In case, take a copy of what you need and full-format (not quick- format) it in your computer and see how it works.
