Lumia 640 asks for PUK unlock code

ATT Microsoft Lumia 640 GoPhone.
Orderer and received unlock code, but as I enter the unlocking code the phone asks for phone PUK unlock code, which I don't have and ATT support doesn't know what am I talking about. When i try to enter wrong unlock code, the counter goes to 4 and it starts from 10 again.
When I enter ##7820# the phone shows like it's unlocked but it won't accept the sim card.

Asking for a PUK code means the sim card is locked by to many wrong SIM PIN attempts. Contact you service provider for your PUK code to unlock the SIM card.
 
I have same error, any luck yet? I read somewhere in a forum about entering a wrong unlock code 10 times to get to a real unlock screen then enter puk code there? have you tried that?
 
Same here. I have a Lumia 735.

The problem is that it keeps 9 attemps left, no matter what. So that's not an option for me.

The unlock code is correct because I dialed ##7820# and unlocked the lock image by entering it. However, the phone still requires the unlock code when I use another carrier SIM. (Works fine with original operator SIM, but is not mine)
 
I couldn't get a result from Microsoft either only a send in which isn't worth it as I'm in New Zealand and I would have to send it by my own cost back to USA.
