Phone App Constantly Displaying Call Notifications

The Phone app seems to almost always have the little red 1 on it, even though I did not receive any calls. Even when I open the app to check, there are no missed calls, so what is it trying to notify me of? Is some kind of spam blocker blocking a call and still showing a notification? It seems pointless to see the notification if I did not actually receive any calls. What is the problem?

Perhaps you have a voice mail? I'm not using a Duo but my Pixel 4 XL will often do that.
