UWP Windows Central App?

IS there a UWP Windows Central App?

I have the windows 10 app, and the windows phone 8 app but neither work on Continuum. (windows 10 only works on PCs)

Yes, the app's Windows 10 version is an UWP app and it supports Continuum. The app is available on both PC and mobile. When you use Continuum on your phone, is the app greyed in the app list or does it crash on launch? Also, what phone do you use?
