Health and fitness has been deprecated.Microsoft to discontinue MSN Food & Drink, Health & Fitness, and Travel apps | Windows Central

On Phone, use Microsoft Health App and Health Vault.

On PC use Health Vault App and sign in to your Health Dashboard via browser.
