I've noticed a bug with Windows 10 Mobile where sometimes when the headphones are removed the volume bar still shows that the headphones are connected, this app will reset the volume bar back to the original Ringer + Notifications and Media + Apps.
Very simple app in future updates it will be ported back to WP 8.1 as I've noticed some threads with people having this problem as well and a live tile to perform the task without having to open the app.
