All accounts are deleting on 640!

I t received a Microsoft Lumina 640 from T-Mobile yesterday. I restored a backup initially which worked fine then woke up and all the accounts on the device (Microsoft, twitter, LinkedIn, exchange) were gone! I did a manual setup of the device and added the Microsoft Account, twitter, LinkedIn and exchange accounts and they are gone again. Anyone else having this issue?
