The biggest improvement I found was doing "improve recognition" as many have, but I found I was looking at the "red light" on the left side of "Microsoft" on the phone. It works much better (at least for me) if I look directly at the Iris camera which is actually on the Right side of "Microsoft"...

I am having the same problems. Tried both exchanging phones and SIM card. I have a case into AT&T to a second level tech so hopefully they will find the problem.
