This bug arises only when we play the songs downloaded from Nokia Music but vanishes if we open the same songs from Nokia Music. So a temporary solution can be to open the songs from Nokia Music until Microsoft comes with a more permanent solution.
This happens on my Lumia 1020 when listening to Podcasts or Music. Its happened a few times. I can't say for sure when it started because the phone is new and only in the last week I've started listening while working out. I've been powering off and back on to fix it.

since you have already faced this issue, how consistent is it in occurence? any particular set of actions that sets this bug in action, how many times have you had this issue, frequency???
I've been having this problem on my 920 for a while now. Only on podcasts. Its definitely the player. Its happening right now, actually. I have the volume up high and the podcast's volume is stuck low. The clicking noise as I am typing this reply (while the podcast is playing) is very loud and responds to me changing the volume. I've fixed this problem in the past by selecting another song and then selecting the podcast again. Unfortunately, my experience has been that the podcast remains stuck at the volume that was set when I continue the podcast. I'm hugely annoyed with this bug as I listen to lots of podcasts. This has never happened with the Podcasts! app by the way.
