Cortana isn't remembering inner circle

I have programmed my wife as part of my inner circle numerous times. I have also entered "wife" and "my wife" as her nickname both manually and when prompted by Cortana when I say "call my wife" (she responds with tell me who your wife is and I'll remember it next time). Cortana frequently forgets in both instances.

The result: My wife doesn't break through during quiet hours and I can't contact her by nickname. Anyone have this issue? Any solutions?

Since last August, have you found a solution to this problem or at least what is causing it? I am still experiencing this as well and have not been able to figure out the cause.

For me, all of my contact data is stored in my Hotmail account, and I do not use Verizon's or any other service for the information. My Hotmail and Facebook are linked.
