Have you tried:
1. restarting the phone / performing a soft reset? (hold power + volume down for 10-15 seconds, or until the phone vibrates)
2. going into settings > applications > photos+camera > reset camera?
3. using a different camera app?
If none of those help, we may have to erase the phone, since that will rule out software. If it persists after a hard reset (settings > about > reset phone) or using the Lumia Software Recovery Tool (on the PC) than we are likely looking at some unfortunate hardware failure.
