One of the things that has bothered me since I got a WP is that when you use a voice command, and now Cortana, all you hear, if you are lucky, is a noise (is it a click, a clunk?). When using Bluetooth, I often miss the sound and have to try, again. Every other phone that I have had would say 'say a command', or such. It would be nice if this were an option for Cortana.

umm, i dont really understand what you are trying to say, that click basically means "say a command", the clunk is when she stops listening. if you can't hear it you could turn up the volume on media + apps.

A quick click is not sufficient as you can miss it, if distracted, especially while driving. I have tried upping the volume. I guess I was spoiled by every other phone that I have owned.

Sometimes I have that problem but more often than that I wish that could give the command more quickly and sometimes even the confirmation bothers me. I think that it depends if you are ina hurry or not.
