To rule out a hardware fault...

Clean the screen over the sensor (you probably already did, I'm just being comprehensive). If a screen protector covers the sensor area remove it or trim so there is a cutout over that area.

Still? Soft Reset and test again. (Soft Reset is holding the Volume Down + Power buttons until the phone vibrates and reboots.) Softy Reset does not cause you to lose data, but can clear up glitchiness by cleaning out system memory.

Still? Make a backup and then Factory Reset (Settings > about > reset your phone). Plug into AC as you will need the battery to stay powered. This will completely reset the phone and put it back to the last update you had and you will restore your backup during the setup process after signing into your Microsoft Account.
