Music Playlists - Songs Keep Getting Deleted

Every time I make a music playlist it eventually becomes devoid of songs. I'm using the native app on a Lumia 635 w/Windows 8.1 & Cyan. I have the music stored on my SD Card (Class 10) - I can press on Album>Add To.., I create a new playlist, I can initially see the correct number of songs that I selected and then:
a few minutes later, the song count reverts to 0.

Can anyone explain or help me on this?

No, in fact it's a brand new Sandisk Ultra Class 10, 32 Gb - of course you never know - I may try another one though just in case.

Do a fresh format using the phone. I find that it works better then doing it on the pc
