Not able to format or update version of windows 10 on lumia 540.

Recently i upgraded my lumia 540 from windows 8 to windows 10 and then updated it to anniversary update i.e. version 14393.67. When the next update came, the update downloaded and when the restart message appeared, i restarted the phone. Then windows logo appeared and then phone started (without the grinding gears) and then message appeared "The phone has been updated but somethings not right". And then when i checked the version, it still displays 14393.67. I tried many times, did soft reset. I tried then hard reset but phone is also not resetting. What should i do?

yesterday, I got the update 10.0.14.393.576 for my 950xl. you can try updating now. if it doesn't work, again reset your phone, this time, using the Lumia recovery tool. it should work.

I already tried to update to 576 version but again it is same problem. So now i will try to reset it using lumia recovery tool as you said.
