SD Card scan issues errors--is the card bad?

some time back my phone nokia lumia 720 displayed the error of SD Card Scan issues. i format the sd card ( in formatting process there was no error) via laptop and also by phone but now my phone can't play the music which i have transferred from laptop and downloaded from nokia mix radio on sd card ( I backup the music before formatting) when i try to play the music it says opening and after sometime screeching voices come out from the phone instead of playing music file. All these music files are playing fine on laptop. Also i found that photographs clicked by the phone have problems to save in SD Card as it doesn't display the snaps after clicking. Is my SD card faulty?

Restart your phone, sd card error will appear and it will ask you to scan, click yes and just wait for about 10-15mins... While its scanning don't do anything on your phone.be patient, this will fix the error... It worked for me...
