How exactly should I take a picture to make sure it's properly focused?

Well that sometimes depends on the phone you are using, but a universal method is hold the phone still and very gently tap the screen, it will always take properly focussed pictures.
Nokia phones have camera shutter buttons that are actually very usable, but i use them mostly while taking closeups, half press does the focus, & if i'm satisfied i press it all the way, but you need very steady hands not to shake the phone while pressing the button.

One other thing to note is that where you click on the screen becomes the focal point. This is especially important if you are taking close-up photos (which I tend to do a lot). Here are a couple of samples I took earlier this week:
