Voice recognition broken with Bluetooth car connection

Anyone else have this problem? My voice recognition is absolutely useless when I'm on Bluetooth connected to my car (a 2012 Toyota). I recall it being fine a while back, but now it's unusable. Without a Bluetooth connection voice recognition works great. I'd hoped 8.1 would fix it, but it hasn't :angry:

I had the worst experience with it just a couple days ago (2010 Nissan Altima). It may have been because my phone was in my pocket. I believe it was ok after I took my phone out.

The thing I've noticed, if I get a text, I respond, but the screen does not lock back once Cortana sends the message.
