Where is Lumia 750

There are no plans for a Lumia 750 as far as we know. The only confirmed Lumia is the 650 and this will be announced some where in Februari

I'm a bit confused as well. Back in September of 2015 they were saying Microsoft will produce Mid-range phones (x2) to go along with Flagship and budget models each year. And there is supposedly some enterprise oriented models due.

550 and 650 seem to have budget covered. 950 and 950 XL seem to have Flagship covered. Nothing else is being confirmed.
