Why does my Nokia Lumia 920 volume glitch?

Sometimes when i turn the volume up or down on my phone, it won't stop. This forces me to restart my phone but does not fix the issue.

What OS and firmware version do you have installed? Is it official Cyan, Preview for Developers or both?

Are you using headphones or phone speaker?

It is the developers preview of wp 8.1 and I am not using headphones. I have also notice it happens when i put my phone on the wireless charger

Also when i restart it it comes up with a lightning bolt with a settings icon.

A hard reset might fix it, if it's a software problem. If it doesn't, it's probably a hardware problem.
