Why is my Lumia 820 stuck in headphones when headphones are not even inserted?

When i press the volume button, I see that the volume is in headphones even though I have not inserted headphones. Before it used to work properly, used to show the volume for media+apps and ringer. The problem started last week and now the volume is stuck in headphones.

Have you tried a soft reboot?
Usually that clears it.

Yes try a soft reset.or if possible you do not want to have this problem in near future try hard reset.

Just try to insert it again and pull straight out till headphone icon clears it
