Why is my Lumia 925 taking pictures with a small fog?

Lumia 925 and camera problem

Hello,
A few days ago I have noticed my L925 rear camera takes pictures with a small fog. At first I clean a lens but same problem. Then I noticed that the lens has a very little scratch. Can a small scratch affect a picture quality? How I can fix it?

Do you use a case, and did you take the pictures with flash?
