Also worth trying a dism /online /cleanup-image /restorehealth followed by an sfc /scannow. This is just in case you have a rather strange (but repeatable) problem I am aware of (it is complex to explain here). If you get problems with the dism instruction that cannot be resolved, then you likely have the issue I'm thinking of, but hopefully you don't. Come back to me if you find you have issues with dism.

I've been dealing with the Windows Defender 'needs to scan' notification issue for awhile. Finally found a solution that worked for me.

I also use CCleaner regularly. It turns out there is a bug in CCleaner that causes Windows Defender to 'forget' that it has scanned. CCleaner *might* be deleting a Windows Defender log or something that records it's activity and then triggers the 'needs to scan' notification in the Action center.

Solution: If you use CCleaner, open it and from home page, go to....
- 'Applications' tab
- scroll down to 'Utilities'
- de-select 'Windows Defender' to be cleaned by CCleaner
