Why doesn't Cortana address me by name ?

When going through original setup she asked for that information ?

In your phone Settings > swipe to Applications > Cortana (or Cortana > settings from search), you will see the option to change your name (or set it if you haven't yet) and then to have Cortana call you by your name.
