UPDATE: As of 12/27 I hadn't received the card, so I called the Citi Prepaid Services phone number that was in the confirmation e-mail. I was redirected to call a separate phone number for cards that were lost, stolen or not received. The CSR cancelled/deactivated the first card and is sending out another card to me, which she said should arrive in 7-10 business days.On 11/24, I filled out the online application and attached photos of the receipt and proof of purchase.
On 12/6, I received an e-mail from Citi Prepaid Services stating that I would be receiving the card within 7-10 business days.
Still waiting for the card, but according to the e-mail, it should arrive no later than 12/20.
