Audio mono

I have a problem when playing music that sometimes the audio will go into mono.
if i change music tracks it usually fixes it for a while, so does a reboot, but the problem always comes back after a few days.
anybody else noticed this?

ive tested with 3 different sets of headphones, still same problem.
