Contacts corrupted!

I have a Lumia 635. My Contacts keep getting messed about - sometimes they get doubled up, sometimes they vanish.

Also, if I add a new contact, I'm asked "to outlook or outlook.com?" Whats the difference, and does it matter?!

for the record my desktop runs Windows 10, and the phone 8.1.
