Do I still need to set the region to USA?

For the Windows 10 Preview, yes.

For Windows Phone 8.1, yes.
For Windows Phone 8.1 Update 1, no it has stuff for the UK.

I have updated my phone and its now current, I can see Cortana however I get this message, "I'm afraid I'm not available to help in your region with the language you have chosen" , My language and region are both UK, in ''settings'' , ''applications'' , ''cortana'' Cortana is disabled due to your combination region and language settings not being supported.

In ''about phone'' it says I have windows phone 8.1 update, how do I get 8.1.1?

Hi had this same issue. What I did was in settings I made sure that language uk was the default (everything active and was above english usa), region format and country said uk both options (phone made me restart) keyboard uk was at the top and language uk was set as the default. In speech uk was set as default. After that I think I just restarted the phone (maybe soft reset...but try a simple power off first) and everything was fine. Let me know if this works.
