I've activated/deactivated it at least three times, so I'd say no.

In my experience, Cortana appears in the app list even when other settings are misconfigured (like having location services deactivated). That she's missing from the app list is what I find most unusual.

This is very frustrating, i didn't find Cortana on app list nor on application settings

If I were you I'd probably try out the official MS support site. This is the site for the UK:

Support | Windows Phone How-to (United Kingdom)

There's a "contact us" button at the bottom. I'd try that out and see if you get any further. Let us know what you find out! :wink:

Still not getting proper answer when i contacts Microsoft. Any suggestions to get my Cortana back?
